An application form that has been signed by your veterinarian

Please print and bring:
             Equipment
Your leash cannot be longer than 4 feet.

Acceptable dog collars are:
  • Flat collar
  • Head harness accompanied with a flat collar
  • Body harness accompanied with a flat collar
  • Martingale
  • Choke Chain
Prong (pinch) and electric collars are not allowed.
The Montana T-Dogger's tester/observers test and observe teams to become registered with Therapy Dogs Inc.   The following must
be given to the t/o before the test can be given.  If we don't have this information when we meet with you to begin the testing/observing
process, you will be rescheduled.
